1. Does wound care connect to the rest of the resident's care?

Many chronic wounds have underlying medical causes.

Your wound care provider should work closely with primary care, podiatry, behavioral health, and your nursing team—not function as an isolated service.

2. What happens between visits?

Wounds don't wait until the next scheduled rounds.

Ask how your nurses reach a clinician when a wound changes and whether telehealth or other clinical support is available between visits.

3. Is documentation survey-ready?

Documentation should be timely, compliant, and available in your EHR without additional work by your staff.

Ask to see a sample note.

4. Is prevention part of the program?

The best wound care programs prevent wounds as aggressively as they treat them.

Ask how the provider identifies high-risk residents and supports pressure injury prevention.

5. Will your staff become stronger?

Your nurses provide wound care every day.

Look for bedside teaching, in-services, continuing education, and certification opportunities that build clinical confidence over time.

6. Can they manage complex wounds?

Routine wounds are only part of the job.

Ask what wounds or skin conditions they don't manage—and what happens to those residents.

7. Can they show building-level data?

Individual notes aren't enough.

Look for reporting that supports QAPI, tracks trends, and helps prepare for survey.

8. Will they support you during survey?

Survey readiness should be built into the partnership.

Ask what support is available if documentation or wound management is questioned during survey.

9. Are they a vendor—or a partner?

Some providers perform weekly rounds.

Others become an extension of your clinical team.

Ask how they define success and how they'll measure it.
